Shoulder and Arm Pain After a Car Accident: Why It’s Often Overlooked
After a car accident, most people expect neck and back pain. What catches them off guard is the shoulder and arm pain that develops in the days that follow — sometimes as a dull ache, sometimes as sharp pain with arm movement, sometimes as numbness or tingling that runs down into the fingers.
Shoulder and arm symptoms after a collision are frequently dismissed or attributed to something other than the accident. They shouldn’t be. These symptoms are among the most clinically significant post-accident findings, and they point to injury patterns that require proper evaluation and documentation.
What’s Actually Causing the Pain
Shoulder and arm symptoms after a car accident typically trace back to one of three sources, and distinguishing between them matters for both treatment and your PI record.
Cervical nerve root involvement. The nerves that supply sensation and motor function to the arm, hand, and fingers exit the spinal cord through openings in the cervical vertebrae. When a collision causes disc herniation, foraminal narrowing, or vertebral misalignment in the neck, those nerve roots can be compressed or irritated. The result is radiculopathy: pain, numbness, tingling, or weakness that radiates from the neck down through the shoulder and arm — sometimes all the way to specific fingers.
Cervical radiculopathy is a serious finding. It does not resolve with rest alone and requires targeted clinical management.
Direct shoulder joint injury. During a collision — particularly a side impact or a crash where the arm braced against the steering wheel or door — the shoulder joint itself can be injured. Rotator cuff strains, labral tears, AC joint sprains, and shoulder impingement can all result from accident forces. These injuries cause localized shoulder pain that worsens with specific arm movements and may limit your ability to reach overhead or across your body.
Thoracic outlet syndrome. The thoracic outlet is the space between the collarbone and first rib through which nerves, arteries, and veins pass on their way to the arm. Accident-related muscle spasm and postural changes can compress this space, producing symptoms that mimic cervical radiculopathy — numbness, tingling, and heaviness in the arm and hand — but originate at a different anatomical location.
Why These Symptoms Are Underreported
Shoulder and arm symptoms after an accident are underreported for several reasons.
First, patients focus on neck and back pain and don’t mention arm symptoms unless specifically asked. Our intake process asks about every area of the body systematically — not just where it hurts most.
Second, the symptoms can be intermittent or positional early on. Numbness that only occurs when driving, or shoulder pain that only appears with certain movements, may not seem significant enough to mention. It is.
Third, adrenaline and the general shock of an accident can suppress awareness of subtler symptoms in the immediate aftermath. Patients often notice shoulder and arm involvement only after the acute neck and back symptoms have been addressed and they begin to move more normally.
Any symptom that wasn’t there before the accident — regardless of how minor it seems — belongs in your medical record.
What a Post-Accident Shoulder and Arm Evaluation Looks Like

- Cervical orthopedic testing to identify nerve root involvement (Spurling’s test, upper limb tension tests, cervical distraction)
- Shoulder orthopedic testing to assess rotator cuff, AC joint, and labral integrity
- Dermatomal sensory mapping to identify specific nerve root patterns
- Motor strength testing of the arm and hand
- Range-of-motion assessment of both the cervical spine and shoulder joint
- Digital X-rays of the cervical spine and/or shoulder if indicated
- Referral for MRI if disc herniation or rotator cuff tear is suspected
Every finding is documented in clinical language that connects the symptom pattern to the accident mechanism. If your symptoms suggest cervical disc herniation, that notation in your record is significant — both medically and for your PI case.
Shoulder and Arm Symptoms Don’t Always Get Better on Their Own
Cervical radiculopathy, rotator cuff injury, and thoracic outlet syndrome are not conditions that typically self-resolve. Without proper treatment, they can become chronic — progressing from intermittent discomfort to persistent pain, functional limitation, and, in some cases, permanent nerve damage.
Early evaluation and treatment matters. And because many of the most significant shoulder and arm injuries don’t produce obvious symptoms in the first 24 hours, a post-accident evaluation is warranted even if you feel “mostly okay.”
