Headaches After a Car Accident: What Causes Post-Traumatic Headaches and When to See a Chiropractor
Headaches are one of the most commonly reported symptoms after a car accident — and one of the most commonly dismissed. Patients assume they are stress-related, take over-the-counter pain relievers, and wait for them to pass. In many cases, that assumption is wrong, and the wait causes real problems both medically and for any personal injury claim that follows.
Post-traumatic headaches after a car accident have specific, identifiable causes. Understanding what’s driving the pain leads directly to more effective treatment and more complete documentation.
The Most Common Causes of Post-Accident Headaches
Cervicogenic headache is the most frequent type we see after motor vehicle collisions, and also the most commonly misunderstood. The name means the headache originates in the cervical spine — not the head. Injury to the upper cervical joints, muscles, and soft tissues refers pain upward into the base of the skull, the occipital region, and often forward into the temples and behind the eyes.
Cervicogenic headaches typically worsen with neck movement, sustained postures (like looking at a phone or computer), and palpation of the upper cervical muscles. They respond well to chiropractic care targeting the cervical spine — because the headache is a cervical spine problem presenting as head pain.
Muscle tension headaches following a collision are driven by sustained muscle spasm in the neck, upper back, and suboccipital region. The muscles that contracted forcefully during impact often remain in protective spasm, generating continuous tension that refers into the head. These headaches feel like a band of pressure around the skull and are often accompanied by neck stiffness and shoulder tightness.
Post-concussion headaches occur when a collision involves a direct head impact — against the steering wheel, headrest, side window, or door frame — or when the rapid deceleration of the brain within the skull produces a concussive event without direct contact. These headaches may be accompanied by cognitive symptoms: difficulty concentrating, light and noise sensitivity, memory issues, and fatigue. Post-concussion headaches require specific evaluation and a different management approach than cervicogenic or tension headaches.
Occipital neuralgia results from irritation or compression of the occipital nerves that run from the upper cervical spine to the back of the scalp. After a collision, inflammation or muscle spasm in the upper cervical region can compress these nerves, producing sharp, shooting, or electric pain in the back of the head and scalp.
Why Headaches After an Accident Deserve a Proper Evaluation
The reason accident-related headaches require clinical evaluation — rather than just time and ibuprofen — comes down to three things.
First, the underlying cause determines the right treatment. Cervicogenic headaches, tension headaches, post-concussion headaches, and occipital neuralgia all have different mechanisms and respond to different interventions. Treating the wrong type is, at best, ineffective and, at worst, makes the underlying condition worse.
Second, headaches can be a warning sign for more significant pathology. New or worsening headaches after a collision can indicate cervical instability, disc herniation with nerve root involvement, or intracranial changes that require imaging. An evaluation determines whether any of these warrant further workup.
Third, undocumented symptoms do not exist in a PI case. An adjuster reviewing your medical record six months after the accident will see only what was documented, and when. A headache that was present from day one but not reported until week four looks — on paper — like it might have started at week four. The clinical record is the legal record.

- Upper cervical examination: palpation of the suboccipital muscles and C1-C3 facet joints, assessment of upper cervical range of motion
- Neurological screening: cranial nerve assessment, cognitive screening if post-concussion symptoms are present
- Cervical spine orthopedic testing to identify structural contributors
- Digital X-rays to assess upper cervical alignment
- Referral for advanced imaging (MRI or CT) if indicated by clinical findings

The Right Time to Come In
The right time to report accident-related headaches is immediately — at your first post-accident evaluation, ideally within 24 to 72 hours of the crash. Even if your headache feels minor, document it. Cervicogenic and post-concussion headaches frequently worsen over the first week as inflammation peaks and muscle spasm deepens.
